When I play ETS2 using the real automatic (keyboard only). I shift into reverse but my truck is unable to reverse at all. Am I doing something wrong?

You can assign up to two keys for one assignment. Go to options > keys & buttons - you will see primary and secondary section. Primary will be the main key for that function and secondary your second. I'm ok with that but doesn't work with a steering Wheel.
